Smyth County Schools and SWVA Community Health Systems School-Based Health will offer athletic physicals to students who plan to participate in Virginia High School League (VHSL) activities for the 2024-2025 school year. Current physicals expire on June 30, 2024; therefore an updated physical must be completed to participate in the next school year.
Physicals will be offered in early May, with exact dates to be announced. Students will report to a specified location at the end of the school day and then be transported to the Saltville Community Health Center. Once seen by a healthcare provider, students will be returned to their school and may drive home or be picked up by a
parent/guardian.
There are no out-of-pocket expenses associated with this service. If applicable, insurance will be filed.
Please complete pages 1, 2, and 4 of the VHSL Athletic Physical form and sign where indicated. Students should return signed forms to school by March 15, 2024.
